CAPS Dental Enrollment Options:

1) Instant Online Enrollment

For instant online enrollment to activate your membership, fill in the Instant Online Enrollment. CAPS accepts the following credit cards: Visa, Mastercard, American Express and Discover Card. Online enrollment is available 24 hours a day, 7 days a week. Complete form details & submit to enroll.

2) Instant phone enrollment – Call (952) 857-1177 Now!

For instant enrollment to activate your membership over the phone contact CAPS Dental Plan at (952) 857-1177. Instant enrollment will require a valid credit card so please have your Visa, Mastercard, American Express or Discover Card available when calling.

Phone enrollment is available: Monday – Thursday 9 – 5 and Friday 9 – 4.

3) Enrollment by mail

Call for an informational plan packet which includes an enrollment form to return with your annual membership dues in the enclosed pre-addressed envelope or you can print & mail us the Online Enrollment form.
